Unless your spouse can read your mind you will never know what's really going on on the inside. You have to trust that your spouse will be open and honest and you have to be prepared for the response. If it is a matter of sexual unfulfillment try something new. Arouse your spouse in unconventional but safe ways. Explore new sex positions that hopefully will give you ultimate pleasure. Be mindful not to indulge in immoral activities. If it is a matter of bad communication determine if there is a miscommunication or mixed communication. While there is no perfect marriage there are sound marriage principles that if followed will result in a great marriage experience. Marriage is hard work, but it is doable.
